A care plan should begin with listening.

Our assessment process helps your family define needs, preferences, safety concerns, and the kind of support that will make the biggest difference.

What to expect

A thoughtful, person-centered assessment.

A qualified member of the Solace Hill care team completes the assessment. An RN may participate in or review the assessment when appropriate, but not every assessment is performed by an RN.

    Tell us what is changing

    We begin with the reason you are seeking help, what has become difficult, and what the client wants to preserve.

    Review daily living needs

    We discuss routines, mobility, personal care, meals, medication reminders, home safety, cognition, and social needs.

    Include key decision-makers

    When possible, the client, family decision-makers, and the person responsible for payment should participate.

  4. 4

    Create the care plan

    We define services, preferences, communication expectations, and next steps before care begins.

Questions families often ask

Assessment is not about judging the home or taking away independence. It is about understanding what support will protect safety, comfort, and choice.

Is this skilled nursing?

No. Solace Hill provides non-medical home care. Assessments are completed by qualified members of our care team. An RN may participate or review when appropriate, but caregivers do not perform skilled nursing procedures.

What should we have ready?

Be prepared to discuss routines, recent changes, mobility, medications that require reminders, safety concerns, preferences, and the desired schedule.

Can the care plan change?

Yes. Care plans should evolve as the client’s needs, goals, and family support change.
